Monthly Membership Benefits:

• Membership includes 4 Botox/Dysport treatments of crow's feet/glabella/forehead, and unlimited touch-ups to these areas*
• 10% OFF any additional Botox/Dysport treatment areas (our regular pricing)
• 10% OFF any filler (our regular pricing)
• 15% OFF skincare products (our regular pricing)
• 10% OFF Hydrafacial or DiamondGlow (our regular pricing)
• Priority Booking
• Advance notice of events or specials

Annual Membership Terms:

• Monthly Fee: $199.00
• Annual Membership Total: $2,388.00
• Cancellation online with $300 fee

Service Terms:

• Appointment required
• Upgrades may be added at time of service for an additional fee
• Additional units will be available at the time of service for a discounted rate
• We reserve the right to refuse membership to anyone at our discretion
• We reserve the right to change the terms of this membership at any time without notice
• Member discounts are not combinable with other promotional discounts

Cancellation Terms:

• Cancel in writing
• 30-Day Notice to Cancel
• Fee Charged if cancelled during first 3 months
• Fee Charged if cancelled during first 6 months
• Membership Fees are Non-Refundable
• This membership fee recurs monthly unless cancelled prior to end of term

Membership Terms and Conditions:

• I understand a 12-month minimum commitment is required and monthly membership dues will be processed every month in the amount of $199.00
• I understand my membership will auto-renew at the end of each one-year term. I agree to provide written notice of cancellation at least 30 days prior to my scheduled auto-renewal
• I authorize Dr. David P. Rapaport & Coolspa to charge my monthly membership fees to my financial institution via Electronic Funds Transfer service, with the credit/debit information that I have provided
• I understand that I am in full control of my payment, and if at any time I decide to make any changes to my credit/debit card information, I must provide Dr. David P. Rapaport & Coolspa with my updated payment method
• I understand my annual membership is non-transferable and may not be shared
• I understand there will be no refunds issued for any charged [member] dues. Transactions will appear as charge from "Coolspa".
• Coolspa reserves the right at any time to cancel, suspend, or refuse membership to anyone at our discretion.
• Coolspa reserves the right to review subscriptions periodically. 
• Members will be given at least 30 days' notice in writing of any changes to membership terms and conditions.
• I, the undersigned acknowledges receipt of Coolspa membership terms and conditions and has read, understands, and agree to be bound by the terms and conditions as part of this agreement

*This monthly membership of $199.00 recurs monthly unless canceled by accessing your online account. A 12-month minimum commitment is required. A $300 early cancelation fee will apply. Service must be used in the month earned or it will be forfeited. We reserve the right to cancel or change this membership at any time without notice. We reserve the right to refuse membership to anyone at our discretion. All treatments and benefits from this membership are non-transferable.
