- This monthly membership is nontransferable and will recur as a monthly payment.
- A 12-month minimum commitment is required.
- A cancelation fee equal to the balance remaining on the 12-month commitment will apply unless proof of relocation or a relevant medical
condition is provided. - We reserve the right to cancel or change this membership at any time without notice.
- We reserve the right to refuse membership to anyone at our discretion.
- All treatments and benefits from this membership are non-transferable.

A year of unlimited Botox®, Daxxify, or Dysport® to the forehead, glabellar, and crow's feet, as well as two Skinvive treatments or four syringes. Skinvive is typically used as two separate syringes, one for each cheek. This package includes two treatments or four syringes to be used at two separate appointments although can be used at your providers' discretion.
